What the physician may need

• Job demands and safety-sensitive duties
• Relevant treatment history
• Current functional assessment

Do not send records or health information through the public partnership form.

Important limits

• Not a universal fitness clearance.
• An in-person assessment or treating clinician may be required.

Coverage and fees

An individual physician-approved quote, only after coverage and payer review.

Check the provincial plan, any employer or insurer payment, program-specific rules and tax treatment. Assessment and documentation must not be double-billed or bundled to conceal insured care.
