Professional partnership enquiries

If activated, the form collects only the professional name, work email, practice name, jurisdiction, role, selected interest and contact consent. Do not submit health information. Professional enquiries would be handled through the operator’s approved enquiry service and internal alerts—not a medical-record system.

Enquiry collection is not yet activated

The responsible legal operator, privacy contact, retention period, approved vendors and intended CRM routing must be finalized before the contact route can accept submissions. The current endpoint does not forward enquiries while that route is disabled.

Technical requests

The hosting provider may process routine network information such as an IP address to deliver and protect the website. Hosting location and provider terms must form part of the operator’s final privacy review; Canadian-only data residency is not claimed.

Before a clinical service opens

A separate clinical privacy and consent process must identify the health-information custodian, access and correction rights, retention obligations, safeguards, processing locations and complaint channel under the applicable jurisdiction’s rules. This public notice is not a substitute.
