The proposed patient pathway

A document follows
a clinical decision.

The service model is designed to keep clinical judgment, patient consent and appropriate follow-up at the centre. Patient appointments are not yet available.

01

Confirm the purpose

Check whether documentation is needed, whether the recipient accepts a self-declaration, and the exact form or evidence required.

02

Confirm location and authority

Identify the patient and their physical location. Assign only a physician authorized and appropriately qualified for the encounter.

03

Review consent, coverage and fees

Explain the care relationship, virtual-care limits, disclosure scope, insured alternatives, fees, tax and relevant terms.

04

Assess—not automatically approve

The physician decides whether a virtual visit, records, physical examination or specialist input is needed. They may decline an unsupported document.

05

Issue an accurate, limited document

Where appropriate, the physician signs a factual document and records the clinical basis and consent in the approved medical-record system.

06

Provide delivery and follow-up

Use the reviewed secure clinical channel, explain recipient limits and correction procedures, and arrange needed follow-up or escalation.
