What the physician may need

• Patient-approved disclosure scope

Do not send records or health information through the public partnership form.

Important limits

• A generic online interview may not establish physical capacity.
• Keep diagnosis disclosure to what is necessary and consented.

Coverage and fees

An individual physician-approved quote, only after coverage and payer review.

Check the provincial plan, any employer or insurer payment, program-specific rules and tax treatment. Assessment and documentation must not be double-billed or bundled to conceal insured care.
