Medical record summary

A factual summary of an existing record for a defined purpose.

Assessment pathway

Authorized records review

Suitability is determined by the physician. A virtual encounter is not a substitute for a needed examination, access to relevant records or a designated practitioner.

What the physician may need

  • Authority to access the source record
  • Purpose and recipient
  • Consent

Do not send records or health information through the public partnership form.

Important limits

  • Not an independent diagnosis or opinion.
  • Clinical transfer, patient access and third-party report fees have different rules.
  • Do not relabel a records-access request as a paid summary. Quebec prohibits fees for preliminary review, sorting or transcription associated with access. A new third-party report needs separate purpose and payer review.

Coverage and fees

An individual physician-approved quote, only after coverage and payer review.

Check the provincial plan, any employer or insurer payment, program-specific rules and tax treatment. Assessment and documentation must not be double-billed or bundled to conceal insured care.
