Suitability is determined by the physician. A virtual encounter is not a substitute for a needed examination, access to relevant records or a designated practitioner.

What the physician may need

• Current Service Canada form
• Treating evidence
• Program and payer requirements

Do not send records or health information through the public partnership form.

Important limits

• Not a general note or guaranteed benefit.
• Confirm any government payment for the report before billing.
• Service Canada currently pays up to C$175 for the ISP-2519 medical report. Confirm the program payment and any permitted patient balance; never bill the same work twice.

Coverage and fees

An individual physician-approved quote, only after coverage and payer review.

Check the provincial plan, any employer or insurer payment, program-specific rules and tax treatment. Assessment and documentation must not be double-billed or bundled to conceal insured care.
