Records and individual assessment

Suitability is determined by the physician. A virtual encounter is not a substitute for a needed examination, access to relevant records or a designated practitioner.

What the physician may need

• Institution’s form
• Relevant longitudinal history
• Functional assessment

Do not send records or health information through the public partnership form.

Important limits

• A new diagnosis or specialist assessment may be needed.
• No guaranteed eligibility.

Coverage and fees

An individual physician-approved quote, only after coverage and payer review.

Check the provincial plan, any employer or insurer payment, program-specific rules and tax treatment. Assessment and documentation must not be double-billed or bundled to conceal insured care.
