Provincial & territorial guidance
Ontario
Local documentation considerations for OHIP and CPSO.
Fees and coverage
Fees for genuinely uninsured documents must be reasonable and disclosed in advance. Insured care and essential elements cannot be privately charged.
Confirm the actual document, insured assessment, public-plan eligibility and any other payer. No patient fee is approved simply by selecting a province.
Physician authority and care
Ontario patients generally require a CPSO-registered physician; narrow best-interest exceptions are not a national operating licence.
Verify the clinician’s current registration, identity, competence and patient-location rules. Plan for examination, follow-up and local urgent care when needed.
Workplace documentation
Medical notes cannot be required to establish ESA sick-leave entitlement. Accommodation, return-to-work and contractual benefits are separate questions.
